Parents/Coach: After completing the form, present it to the WRSA office with your player card. Releases may take up to 72 hours to complete. Parents are encouraged to provide as much notification to their current club team and registrar to complete proper paperwork to prevent delays. Players who have not met all of their financial obligations to their current team/club WILL NOT be approved for drop or transfer until all financial matters are settled or PAID IN FULL.

Releases will not be finalized until the following actions are taken:
- This form is completed with all appropriate signatures and information.
- Return of the current player card to the registrar from the current team.
- Signature of the current registrar.
